COVID-19 Student Rule Violations - Referral Form


In accordance with campus rules applied to address COVID-19 safety concerns, UT Dallas community members should use this form to notify the Office of Community Standards and Conduct of student incidents of non-compliance, such as refusing to wear a face covering, not complying with social distancing rules, and self-reporting of COVID-19 diagnosis, symptoms consistent with COVID-19, or exposure to someone who is COVID-19 positive.
